Optimal treatment of patients with chronic coronary syndromes
Current 2019 ESC Guidelines for stable coronary heart disease emphasize dynamic nature of the disease, introduce the new term „chronic coronary syndromes“ and distinguish six possible scenarios. Therefore, optimal treatment is not the same for every patient, but should be adapted to specific clinical scenario and individualized according to each patient's specific characteristics. Such treatment should include healthy lifestyle measures, combination of appropriate drugs, and in some cases myocardial revascularization. Pharmacological treatment consists of anti-ischaemic drugs, drugs for the cardiovascular events prevention, and optimal control of main coronary risk factors (arterial hypertension, dyslipidaemia, diabetes). The need for percutaneous or surgical myocardial revascularization is based on the existence of anginal symptoms, non-invasively documented ischaemia and the localization, degree and functional significance of coronary stenosis on invasive coronary angiography.
