Enterosgel – medicinski proizvod s potencijalno širokom primjenomEnterosgel - a medical product with a potentially wide range of applications

Medicinski proizvod klase IIa Enterosgel hidrogel je baziran na polimetilsiloksanu koji se dobiva polikondenzacijom metilsilikatne kiseline uz gubitak vode i formiranje siloksanske veze (Si-O-Si). Sušenjem organosilikatnoga gela nastaje kruti mezoporozni adsorbent (kserogel) sa specifičnom površinom od 300 m2g-1. Udio kserogela u medicinskom proizvodu Enterosgel je oko 7%. Proces adsorpcije putem Enterosgela odvija se dvama mehanizmima – molekularnom adsorpcijom i kosedimentacijom u gelu. U usporedbi s aktivnim ugljenom koji se najčešće primjenjuje kao oralni adsorbent (enterosorbent), Enterosgel ima niži kapacitet prema spojevima s molekularnom masom manjom od 1500 Da, ali je mnogo potentniji adsorbent od aktivnog ugljena prema sposobnostima vezanja spojeva velike molekularne mase poput proteina i bakterijskih endotoksina. Brojnim eksperimentalnim i kliničkim ispitivanjima istražena je oralna primjena Enterosgela u liječenju gastroenteroloških bolesti, bubrežne insuficijencije, šećerne bolesti, hiperlipidemija, alergijskih stanja poput astme, ekcema i alergijskog dermatitisa, teških opeklina, politraume, sepse, multiplog zatajenja organa, u prijeoperacijskom razdoblju s rizikom za gubitak velike količine krvi, u onkologiji s ciljem smanjenja nuspojava kemoterapije te kod raznih intoksikacija (teški metali, organofosforni pesticidi, profesionalna otrovanja – benzen, cikloheksan).

Medical product Class IIa Enterosgel hydrogel is based on polymethyilsiloxane that is obtained by polycondensation of methyl silicate acid, with a loss of water and formation of siloxane bonds (Si-O-Si). Through the drying of organic silica gel the solid mesoporous adsorbent (xerogel) is produced, with a specific surface area of 300 m2 g-1. The portion of xerogel in the medical product Enterosgel is about 7%. The process of adsorption by Enterosgel takes place by two mechanisms - molecular adsorption and co-sedimentation in the gel. Compared with activated carbon, which is usually administered as an oral adsorbent (enterosorbent), Enterosgel has a lower capacity to the compounds with a molecular mass of less than 1500 Da, but is a far more potent adsorbent for binding to high molecular weight compounds, such as proteins and bacterial endotoxins, than the activated carbon is. Numerous experimental and clinical studies investigated the oral administration of Enterosgel in the treatment of gastrointestinal diseases, renal failure, diabetes, hyperlipidemia, allergic conditions such as asthma, eczema and allergic dermatitis, burns, polytrauma, sepsis and multiple organ failure in the preoperative period with the risk of losing large amounts of blood, and in oncology in order to reduce side effects of chemotherapy and various intoxications (heavy metals, organophosphate pesticides, occupational poisonings - benzene, cyclohexane).
