Palijativna skrb u obiteljskoj mediciniPalliative care in family medicine

Liječnik obiteljske medicine, sa suradnicima i obitelji bolesnika, nositelj je palijativne skrbi u primarnoj razini zdravstvene zaštite i to je jedan od njegovih najkompleksnijih zadataka. Ima prosječno 9 bolesnika godišnje kojima treba palijativna skrb, od kojih će polovina umrijeti zbog raka i ti će bolesnici 90% vremena posljednje godine života provesti u svom domu u skrbi tima obiteljskoga liječnika te obitelji. Sadržaj, intenzitet i trajanje palijativne skrbi u obiteljskoj medicini ovisit će o bolesti, stanju bolesnika i uvjetima u kojima se skrb pruža. Obiteljski liječnici skrbe o bolesniku kroz sva razdoblja bolesti, u ordinaciji, domu bolesnika, ustanovi za starije osobe. Rješavaju fizičke, psihološke, socijalne i duhovne potrebe bolesnika te u radu primjenjuju holistički pristup. Skrbe o obitelji bolesnika te daju obitelji podršku u žalovanju. U provedbi palijativne skrbi surađuju sa svim sudionicima palijativne skrbi poštujući na znanstvenim spoznajama utemeljenu stručnu podjelu rada, multidisciplinarni pristup i timski rad.

Being responsible for palliative care at the primary healthcare level, together with the associates and the patient’s family, is one of the family medicine physician’s most complex tasks. An average of nine patients seeks palliative care every year. Half of these would terminate due to a malignant disease, spending 90% of their last year at home, being taken care of by their family and the family physician’s team. The content, intensity and duration of palliative care in the family medicine practice depends on the disease, the patient’s status and the conditions in which the palliative care is being provided. The family medicine physicians care for their patients throughout all stages of the disease - at the family medicine office, at patient’s home and in the elderly care institutions. Applying a holistic approach, they deal with physical, psychological, social and spiritual needs of their patients. They also care for the patient’s family and provide the support in mourning. They cooperate with all the stakeholders in the palliative care, respecting the scientific-based professional division of tasks, multidisciplinary approach and teamwork.
